Job summary

Four Pillars Hospice in Texas seeks a Nurse Practitioner to oversee clinical care and coordinate with the hospice physician and IDG/T to ensure quality end-of-life care.

You will conduct patient visits, develop and implement individualized care plans, educate patients and families, and support staff in palliation. Flexible scheduling and local travel are required to meet patient needs and maintain compassionate care.

Qualifications

  • Must hold RN license and NP practice license in Texas.
  • Graduate from a BSN and NP-focused program (MSN/DNP).
  • Two years of advanced practice nurse experience; hospice/home health preferred.
  • Strong knowledge of pharmacology and palliative care.
  • Ability to work flexible hours and travel locally.
  • Commitment to HIPAA and compassionate patient care.

Responsibilities

  • Collaborate with the IDG/T to assure appropriate evaluation and palliative treatment.
  • Consult with and assist in the IDG/T's development of patient/family plan of care.
  • Educate patient/family regarding disease processes and end-of-life care; provide staff training.
  • Perform and document visits to patient/family per plan of care.
  • Interact with attending and consulting physicians regarding palliation and care.
  • Provide education to IDG/T members on clinical issues related to palliation.
  • Actively participate in IDG/T communications and timely updates.
  • Inform physicians of changes in patient needs and outcomes.

Position Summary

The Nurse Practitioner's Primary functions are to assist in overseeing the clinical care and treatment of patients and families; to consult with the patients' attending physician, in consultation with the hospice physician; provide clinical direction to the Interdisciplinary Group/Team to ensure quality care.

Key Responsibilities
  • Collaborate with the IDG/T in assuring appropriate evaluation and palliative treatment of terminal disease.
  • Consult with and assist in the IDG/T's development of patient/family plan of care to assure quality care is provided.
  • Consult with and educate the patient/family regarding disease process, self-care techniques, end-of-life care. Provides training to other staff as needed.
  • Perform and document visits to patient/family based on plan of care and patient/family need.
  • Interact with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physician.
  • Consult with and provide education to other IDG/T members on a prn basis for clinical issues relating to palliation.
  • Activelyparticipate in the IDG/T. Communicate effectively, timely, and appropriately at all times.
  • Inform physicians and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient/family response to care.
  • Complete face-to-face encounters with patients, as assigned, prior to 3rd benefit period and each period afterwards.
  • Provides clinical assessment results to hospice physician after making face-to-face encounters with patients, as assigned.
  • Always uses standard precautions, and per hospice policy.
  • Adhearsto hospice policies regarding protected health information.
  • Keep up with and maintains education/knowledge in pharmacology and palliative medicine.
  • Provide on-call services to patients/families as assigned.
Qualifications
  • Graduate from a Baccalaureate School of Nursing, a Nurse Practitioner (NP) focused Master's or Doctoral Nursing Program
  • Currentstate license as a Registered Nurse, and as an Advanced Practice Nurse. Current driver's license.
  • Twoyears' experience as an Advance Practice Nurse, clinical care setting preferred; hospice or home health preferred.
  • Nursingskills as defined as generally accepted standards of practice. Comprehensive knowledge of pharmacology, palliative care, and end-of-life. Good interpersonal skills..
  • Transportation: Reliable transportation and valid auto liability insurance.
  • Ability to work a flexible schedule
  • Ability to travel locally
  • Ability to remain calm in stressful conditions and make quick decisions to meet patient and family individualized psychosocial needs.
