Job summary

St. David's Healthcare at Home, part of HCA Healthcare, seeks a PRN Home Health Social Worker (MSW) to implement standards of care for medical social work services and collaborate with the interdisciplinary team to plan and coordinate patient care.

Responsibilities include psychosocial assessments, liaison with nurses and community agencies, and development of individualized plans of care with patient and family input to ensure quality home care and regulatory compliance.

Qualifications

• Master’s degree in Social Work or in a human services field (e.g., sociology, psychology).
• Experience in healthcare settings with individuals.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assess the psychosocial status of patients and families/c caregivers.
• Coordinate care with the interdisciplinary team and community agencies.
• Develop and update individualized plans of care with patient/family input.
