Job summary

Trinity Health At Home is seeking a Medical Social Worker (MSW) to provide counseling, care coordination, and community resource support to homebound patients and their caregivers.

The MSW will work closely with physicians and interdisciplinary teams to overcome social and emotional barriers affecting health, safety, and quality of life, with emphasis on compassionate, person-centered care and ongoing documentation.

Qualifications

• Master's Degree in Social Work (MSW) from an accredited program required.
• Current Social Work licensure as required by state regulations.
• CPR certification required.
• Minimum two years of healthcare social work experience preferred.

Responsibilities

• Conduct comprehensive psychosocial assessments and develop individualized care plans.
• Provide counseling and emotional support to patients and caregivers facing illness, disability, or life transitions.
• Connect patients and families with community resources, financial assistance programs, and support services.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care.
• Educate patients and caregivers on coping strategies, available resources, and care options.
• Monitor patient progress and adjust care plans as needed.
• Maintain accurate and timely clinical documentation.
• Participate in interdisciplinary team meetings, care conferences, and quality improvement initiatives.
• Support patients with complex social and emotional needs to improve health outcomes and independence.
