Job summary

Community Health Network in Indianapolis is seeking a Medication Reconciliation Technician to obtain and document medication histories in the EMR accurately and timely, in line with Joint Commission standards.

You will interview patients and families, conduct pharmacy and facility interviews by phone, track interventions, and enter Prior to Admission histories into EPIC. A high school diploma and IPLA Pharmacy Technician licensure are required, with national certification due within two years.

Qualifications

• High school diploma or GED required.
• Licensed as a Pharmacy Technician by IPLA or Pharmacy Intern licensure.
• National certification (PTCB or NHA) within two years of hire.
• If licensed as Pharmacy Intern, national certification may not be required.

Responsibilities

• Interpret and enter Prior to Admission medication history into EPIC EMR.
• Perform thorough interviews with patients and family members.
• Conduct telephone interviews with outpatient pharmacies or long-term care facilities.
• Track interventions and productivity using department processes.
• Communicate regularly and professionally with patients, families and hospital staff.
