Job summary

Community Health Network is seeking Pharmacist student candidates for paid Medication Reconciliation Technician roles. You will obtain and document medication histories in the EPIC EMR and perform patient interviews to ensure accurate histories.

This position requires HIPAA-compliant communication with patients, families, and hospital staff, with flexible scheduling for students and adherence to regulatory standards.

Qualifications

• High school diploma or GED equivalent required.
• Licensed as a Pharmacy Technician by the IPLA or licensed as a Pharmacy Intern by the IPLA (interns not required to be nationally certified).
• Pharmacy Technician, Pharm Tech, Pharmacy Tech.

Responsibilities

• Interpret and enter Prior to Admission medication histories into EPIC EMR.
• Conduct thorough interviews with patients and family members to collect medication histories.
• Perform telephone interviews with outpatient pharmacies, long-term care facilities, or other sources for medication history.
• Track interventions and productivity using the department's process.
• Regularly and professionally communicate with patients, families, and hospital staff.

We are looking for Pharmacist students for our paid Medication Reconciliation Technician positions. Flexible schedules for students. The Medication History Technician will obtain and document medication histories in the electronic medical record in an accurate and timely manner in compliance with standard procedures, the Joint Commission, and other regulatory standards.

Your exceptional skills

Interpret and enter "Prior to Admission" medication history into the EPIC electronic medical record, perform thorough interviews with patients and family members, conduct telephone interviews with outpatient pharmacies, long-term care facilities, or other locations that may provide medication history, track interventions and productivity using department's process, regularly and professionally communicate with patients, their families, and hospital staff

Your exceptional qualifications

High School diploma or GED equivalent required.

One of the following Licensures is required:

• Licensed as a Pharmacy Technician by the Indiana Professional Licensing Agency (IPLA)
• Licensed as a Pharmacy Intern by the Indiana Professional Licensing Agency (IPLA). If licensed as Pharmacy Intern, you do not need to be nationally certified with PTCB or NHA
• Pharmacy Technician, Pharm Tech, Pharmacy Tech
