Job summary

At Avir at Wedgewood in Fort Worth, Texas, we seek a licensed vocational nurse (LVN) to join as a PRN team member. You will provide compassionate nursing care to residents in a warm, supportive environment and collaborate with the broader care team.

Responsibilities include administering medications, charting and record-keeping, assessing residents, coordinating admissions and transfers, and ensuring care plans align with physician orders and regulations.

Qualifications

• Must be a RN in good standing and state-licensed.
• Experience as Charge Nurse or supervisory role preferred.
• CPR certification required.
• Ability to interpret long-term care regulations.

Responsibilities

• Provide nursing care to residents as prescribed by the physician.
• Prepare and review medical records and assess residents.
• Monitor resident activity and evaluate nursing care per physician orders and care plans.
• Coordinate admissions, transfers, and discharge of residents.
• Transcribe physician orders and communicate with residents, families, and team.
