Job summary

The Center at CenterPlace in Greeley, Colorado, seeks team-oriented PRN LPNs to deliver high-quality patient care in a premier short-term rehabilitation setting.

LPNs will administer and document medications, monitor patient status, educate patients, and coordinate with CNAs to ensure care plans are followed, in a hotel-like clinical environment. Valid LPN license is required for employment.

Responsibilities

• Administers and documents medications and treatments per physician orders.
• Evaluates patients routinely and with changes of condition.
• Communicates changes of condition and test results as per policy.
• Educates patients as necessary.
• Ensures the patient care plan is implemented.
• Supervises and coordinates CNAs.
