Job summary

The Center at CenterPlace in Greeley, CO, is seeking a team-oriented LPN for the 6pm–10pm shift on our short-term rehabilitation unit. You will administer medications, monitor patient progress, and deliver compassionate care in a modern, hotel-like setting.

This role involves documenting treatments, communicating changes of condition, educating patients, ensuring the care plan is implemented, and supervising CNAs to maintain safety and quality.

Qualifications

• Current LPN license in Colorado.
• Experience in short-term rehabilitation or skilled nursing preferred.
• Willingness to work 6pm–10pm shifts.

Responsibilities

• Administers and documents medications and treatments per physician orders.
• Evaluates patients regularly and notes changes in condition.
• Communicates changes, test results, and care updates per policy.
• Educates patients as needed about care and medications.
• Ensures the patient care plan is implemented.
• Supervises and coordinates CNAs.
