Job summary

Addus HomeCare in Truth or Consequences, NM is seeking a Licensed Practical Nurse (LPN) to join our home health team. This role provides direct patient care, supports the plan of care, and educates patients and families in self-care.

Requires graduation from an accredited LPN program, current NM license, CPR, and reliable transportation. PRN opportunities with 2-3 days weekly, potential full-time options, and a focus on compassionate, in-home care.

Qualifications

• Must be licensed as LPN/LVN in NM and CPR certified.
• Prior nursing experience in home health or medical/surgical settings preferred.
• Demonstrates ability to document care accurately and timely.

Responsibilities

• Provide direct patient care per Nurse Practice Act.
• Implement and document the plan of care initiated by the RN.
• Educate patient and family on self-care and diagnoses.
• Coordinate with home health team to address patient needs.
• Maintain equipment and utilize aseptic technique.
• Engage in ongoing professional development.
