Job summary

Hendrix Health and Rehabilitation is seeking a compassionate Licensed Practical Nurse (LPN) to join our team on a PRN (as-needed) basis. You will deliver direct nursing care, collaborate with RNs, physicians, and the interdisciplinary team to promote resident well-being in our long-term care and rehabilitation facility.

Responsibilities include administering medications, monitoring residents, documenting care, and supporting admissions.

Qualifications

• Current Licensed Practical Nurse (LPN) license.
• Current BLS/CPR certification required.
• Strong clinical and assessment skills.
• Experience in long-term care or rehabilitation preferred, but not required.
• Excellent communication and interpersonal skills.
• Compassionate, dependable, and resident-focused.

Responsibilities

• Provide direct nursing care to residents per individualized care plans.
• Administer medications and treatments as prescribed.
• Monitor residents and report changes promptly to RN or physician.
• Document assessments, treatments, and resident responses.
• Assist with admissions, discharges, and physician orders.
• Collaborate with the interdisciplinary team to ensure quality care.
• Supervise CNAs and support resident care quality.
• Maintain compliance with state, federal, and facility policies.
• Promote a safe, clean, and compassionate environment for residents and families.
