• Observe and document changes in residents' condition and update nursing plans as needed.
• Coordinate care with the Director of Nursing, physicians, and other healthcare team members.
• Participate in care plan meetings and interdisciplinary team discussions.
• Follow infection control guidelines and HIPAA privacy rules; participate in training and audits.
