Job summary

Qualifications

• Graduate of an accredited school of nursing.
• Current LPN license in the state of practice.
• CPR certification.
• Experience in a long-term care or skilled nursing facility preferred.
• Strong clinical skills with attention to detail and excellent communication.

Responsibilities

• Provide direct resident care per individualized care plans, ensuring comfort and dignity.
• Administer and document physician-ordered medications and treatments.
• Monitor resident responses to medications and treatments; report adverse effects.
• Observe and document changes in condition; update nursing care plans as needed.
• Coordinate care with the Director of Nursing, physicians, and interdisciplinary team; participate in care plan meetings.
• Supervise, plan, and evaluate work of nursing personnel; conduct rounds and report changes.
• Maintain a safe environment; report environmental deficiencies and ensure infection control and HIPAA compliance.
• Perform other duties as assigned.
