Job summary

Elara Caring seeks a Licensed Vocational/Practical Nurse (LVN/LPN) in Louisiana to join our home health team. You will follow patient care plans, monitor progress, update plans, and collaborate with the RN Case Manager to ensure safe, coordinated care in patients’ homes.

Requirements include an active LVN license, graduation from an accredited nursing program, and at least 1 year of clinical experience. Employee benefits include medical/dental/vision, 401(k) with match, tuition reimbursement,

Qualifications

Current State LVN license in the state of Louisiana.
Graduate of an accredited nursing program (certificate, diploma, or associate degree).
At least 1 year of clinical care experience preferred.
Ability to lift/push/pull up to 50 pounds and assist with patient transfers.
Must have reliable transportation or a valid driver’s license for travel between patient homes.

Responsibilities

Follow each patient's care plan and physician orders.
Monitor patient progress and report changes to the care team.
Educate patients and caregivers on self-care techniques.
Update care plans with the RN Case Manager as needs change.
Document medication regimens and maintain medication profiles.
Communicate with physicians, patients, families, and the care team for coordinated care.
Help prevent avoidable hospitalizations through proactive patient care.
Deliver compassionate, high-quality care to support positive outcomes.
