Job summary

Lehigh Valley Companion Care Homes is seeking a compassionate LPN to provide skilled nursing care in clients' homes and community settings on a PRN basis. The ideal candidate will deliver high-quality care while prioritizing health and well-being in a compassionate environment.

Requirements include Pennsylvania LPN license and at least 1 year of home care or related experience; strong communication and ability to work independently; willingness to travel to client homes as needed.

Qualifications

• Current and valid LPN license in Pennsylvania.
• Minimum of 1 year of nursing experience in home care, long-term care, or similar healthcare settings.

Responsibilities

• Provide skilled nursing care to patients in homes and community.
• Administer medications and monitor treatment plans as prescribed.
• Perform health assessments, including vital signs, and document findings.
• Assist with personal care tasks as needed.
• Support patients with mobility and physical therapy exercises to enhance well-being.
• Ensure patient safety and comfort through proper care and supervision.
• Coordinate with family and healthcare providers to ensure optimal care and support.
• Respond to medical emergencies and provide first aid as required.
• Document care provided and maintain accurate patient records.
