Job summary

Del Corazon Hospice is seeking a Nurse Practitioner (NP) to conduct face-to-face visits with hospice patients in their homes or facilities across Santa Fe, NM. You will collaborate with the Medical Director to ensure comprehensive patient care and continuity.

The ideal candidate holds a Master’s or Doctorate in Nursing with NM NP licensure and an APRN certification, plus at least one year of hospice experience and excellent clinical assessment and communication skills.

Qualifications

• Master’s degree or Doctorate in Nursing with current NP licensure in New Mexico.
• Minimum of 1 year of NP experience in hospice or palliative care.
• Current APRN certification.
• Strong clinical assessment and communication skills with empathetic patient care.

Responsibilities

• Conduct face-to-face visits with hospice patients in homes or facilities to assess and support clinical needs.
• Collaborate with the hospice Medical Director and other healthcare professionals to ensure optimal patient care and continuity.
• Evaluate and manage patient symptoms, adjust care plans as needed, and provide emotional support to patients and families.
• Document patient encounters accurately and timely to ensure regulatory compliance.
• Educate families and caregivers about the end-of-life process and available resources.

We are seeking a skilled NP to conduct face-to-face visits and collaborate closely with our hospice Medical Director to ensure comprehensive patient care.

Key Responsibilities

• Conduct face-to-face visits with hospice patients in their homes or facilities to assess and support their clinical needs
• Collaborate with the hospice Medical Director and other healthcare professionals to ensure optimal patient care and continuity
• Evaluate and manage patient symptoms, adjust care plans as needed, and provide emotional support to patients and families
• Document patient encounters accurately and timely to ensure regulatory compliance
• Serve as a resource for clinical questions and assist in educating families and caregivers about the end-of-life process and available resources

Qualifications

• Education: Master’s degree or Doctorate in Nursing, with current NP licensure in New Mexico
• Experience: Minimum of 1 year of experience as an NP in a hospice or palliative care setting
• Certification: Current Advanced Practice Registered Nurse (APRN) certification
• Skills: Strong clinical assessment and communication skills, with an empathetic approach to patient care
• Transportation: Reliable transportation and valid auto liability insurance (mileage reimbursement provided)
