Job summary

RHA Health Services, LLC in Warner Robins, GA is seeking PRN Licensed Practical Nurse (LPN) to work days or nights in a group home setting. Compensation starts at $32/hr+, depending on role and experience, with schedules 7am-7pm or 7pm-7am.

Responsibilities include holistic nursing care, health data collection, coordinating routine medical care, documenting care, assisting physicians, training unlicensed staff, conducting medication passes, QA checks, and ensuring regulatory compliance.

Qualifications

  • Graduation from an accredited licensed nurse program.
  • Current LPN license with no restrictions to practice nursing.
  • Current CPR Certification or eligibility to certify for CPR.
  • Possess credentials required by Federal or State law.
  • Effective communication with Interdisciplinary Team Members and families.
  • Ability to work independently, stay organized, and prioritize tasks.
  • Must be at least 18 years old.

Responsibilities

  • Provide holistic nursing care to individuals with IDD.
  • Observe, collect health data, and report health needs to the RN.
  • Coordinate preventative and routine medical care for people supported.
  • Document nursing care provided.
  • Assist physicians with on-site medical clinics where applicable.
  • Provide health trainings to unlicensed personnel per state regulations.
  • Conduct medication pass observations to ensure staff competency.
  • Perform quality assurance checks and observations for task compliance.
  • Report abnormal health findings to physicians.
  • Communicate with physicians, pharmacies, and the Interdisciplinary Team.
  • Advocate for people supported and assist with community supports.

Job description

We are hiring for: PRN Licensed Practical Nurse (LPN) - Days or Nights

Location: Warner Robins, GA (group home setting)

Employment Type: PRN (as needed)

Compensation: $32/hr+ (depending on role and experience)

Incentives: No sign-on bonus is being offered. No relocation packaged offered.

Schedule: Varies, 7am - 7pm OR 7pm - 7am

Job Responsibilities
  • Provide holistic nursing care to individuals with intellectual and developmental disabilities
  • Observe, assist with gathering health related data, and report the health needs or the people we support to the registered nurse
  • Coordinate preventative and routine medical care for people supported
  • Document nursing care provided
  • Assist physicians with on-site medical clinics where applicable
  • Provide health related trainings to unlicensed personnel according to state regulations
  • Conduct medication pass observations in the home to ensure staff competency according to state regulations
  • Conduct medication closest assessments
  • Perform quality assurance checks and observations to ensure unlicensed personnel are performing health related tasks as assigned according to state regulations
  • Observe for and report any abnormal health related findings to the physician
  • Communicate with physicians, pharmacies, medical suppliers, and the Interdisciplinary Team
  • Advocate for the people supported and assist with developing community supports to promote optimal health for the people supported
Minimum Qualifications
  • Graduation from an accredited licensed nurse program
  • Current LPN license with no restrictions to practice nursing
  • Current CPR Certification or eligibility to certify for CPR
  • Possess credentials required by Federal or State law
  • Must be able to communicate effectively with Interdisciplinary Team Members, as well as the people supported and their families
  • Must be able to work independently, be organized, and possess the ability to prioritize work tasks
  • Must be at least 18 years of age
Physical Demands and Work Environment

Must be able to regularly lift and carry up to 20 lbs. and occasionally pull at least 40 lbs. Must be able to squat, kneel, crawl, crouch, climb, and stoop as part of regular job duties. Must be able to use hands and fingers to handle or operate objects, tools, or controls. Required to stand and walk for extended periods. Must be able to demonstrate proficiency in CPR from floor level, which requires working on hands and knees, bending, standing, and lifting. Vision requirements include close, distance, and peripheral vision. Must be able to talk and hear. Exception – Deaf and Hard of Hearing Programs: For these programs only, employees must be fluent in American Sign Language (ASL). The requirement to talk and hear does not apply.

  • Drug testing Education verification and other credentialing based on position requirements.
  • Proof of employment history or references (if required)
  • Position that requires driving Proof of driver's license, driver's insurance, and vehicle, IF required for providing transportation for individuals
