DODGE COUNTY HOSPITAL is seeking a Licensed Clinical Social Worker (PRN) for the Geriatric Behavioral Health Unit. The role involves providing social service functions, including assessments, treatment plans, and case management for patients and families.

The ideal candidate will have a master’s degree in social work and current licensure such as LCSW or LPC, along with experience in a psychiatric setting. This position offers flexible hours in a supportive environment.

Formación

• Master’s degree in social work or clinical nurse specialist for psychiatric mental health.
• Current licensure such as LCSW or LPC experience in psychiatric setting preferred.
• Knowledge of community resources for older persons.

Responsabilidades

• Provide assessments, treatments, and case management to patients and families.
• Complete assessments such as Suicide Risk Assessment and Psycho-Social Assessment.
• Participate in weekly treatment team meetings and document follow-ups.
