Job summary

Deer Oaks is seeking a Licensed Clinical Social Worker (LCSW) to join our geriatric behavioral health team, working onsite in long-term care facilities. The role offers clinical autonomy, a predictable Monday–Friday schedule, and robust administrative support to focus on patient care.

You will provide therapy, assessments, and care planning for residents, collaborating with interdisciplinary teams while maintaining compliant documentation and high-quality care.

Qualifications

• Active, unrestricted LCSW license in the state.
• Valid driver’s license and reliable transportation.
• Strong written and verbal communication skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Commitment to timely documentation and high-quality care.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Support residents with depression, anxiety, adjustment to illness, grief, trauma, substance use, and other LTC-related conditions.
• Deliver behavioral management and brief, goal-oriented interventions aligned with geriatric care best practices.
• Work closely with nursing, social services, rehabilitation, and medical providers to coordinate treatment.
