Job summary

Advocate Health is seeking a PT for PRN opportunities in Inpatient Rehabilitation with locations at Carolinas Rehabilitation Northeast and Carolinas Rehabilitation Mount Holly. The position requires a PT license, CPR/BLS certification, and a Bachelor's degree in Physical Therapy.

Qualifications

• Minimum Bachelor's Degree in Physical Therapy from an accredited program.
• Active Physical Therapist license in the state.
• CPR Certification and BLS per policy guidelines.

Responsibilities

• Evaluates patients and develops an appropriate care plan including modalities and exercise programs as warranted by the patient's diagnosis and condition.
• Gathers and interprets data from medical records, physician reports, patient, and/or family and identifies needs for assistive devices.
• Documents evaluations, tests, progress notes, daily treatment notes, attendance records, education, outcomes, discharge notes, and charges.
• Re-evaluates patients as needed, adjusting goals as status changes.
• Communicates evaluations and progress to physicians and other staff and coordinates scheduling and discharge planning.
• Provides patient/family education on deficits, progress toward goals, and home program guidance including equipment use.
• Directs and supervises PTA, PT Technician, students, and volunteers and assists with orienting staff.
