Job summary

CarePartners Health Services in Asheville, NC seeks a Clinical Rehab Specialist PRN to coordinate admissions for inpatient rehabilitation. You will work with case managers, physicians, and payors to ensure timely patient placement through clinical screenings.

In this role you will partner with Rehab Medical Director and hospital staff to maintain a smooth referral flow and exceptional patient care in a fast-paced post-acute setting.

Qualifications

• Licensed Professional in a clinical health care specialty (i.e. RN, Physical Therapist, Occupational Therapist, or Speech Therapist).
• Current state licensure in clinical practice area or eligible for state license transfer.
• Graduate of an accredited school in clinical specialty.
• Minimum 2 years of recent clinical care experience.

Responsibilities

• Coordinate between internal and external referral sources and inpatient rehab operations.
• Establish relationships with case managers, physicians, payors, and other referral sources.
• Ensure appropriate patient placement through clinical screenings and assessments in collaboration with Case Management and Rehab Medical Director.
• Support admissions process for inpatient acute rehab program with hospital case management.
