Job summary

Active In-Home Therapy is seeking a Home Health Speech Therapist to provide in-home speech and swallowing therapy. You will assess patients, design individualized treatment plans, and deliver therapy in a home setting, working within a supportive healthcare team.

This PRN role offers flexible scheduling and a W2 employment structure, with opportunities to cover preferred areas and contribute to improving patients’ quality of life.

Qualifications

• Current state licensure as a Speech-Language Pathologist required.
• Home health or related setting experience preferred but not required.
• Strong clinical and critical thinking skills are essential.
• Excellent communication and interpersonal abilities required.
• Ability to work independently and with a healthcare team.

Responsibilities

• Conduct comprehensive assessments of speech, language, and swallowing disorders to determine therapy needs.
• Develop and implement individualized speech therapy treatment plans with the healthcare team.
• Provide hands-on speech therapy interventions to address communication and swallowing challenges.
• Educate patients and families on speech and language techniques to improve function.
• Monitor and document progress and adjust plans as needed.
• Collaborate with physicians, nurses, and other therapists for holistic care.
• Maintain accurate and up-to-date patient records.
• Communicate effectively with patients, families, and caregivers to foster a supportive environment.
