Job summary

Transitions At Home, a Wisconsin-based nonprofit home health provider, seeks a PRN Speech Language Pathologist to deliver intermittent in-home therapy under physician orders. You will work with a compassionate team in patients' homes, following a plan of care and supervision by the Clinical Director.

Flexible schedule, competitive hourly wage of $45-$50, and benefits including 401k, FSA, tuition reimbursement, and referral programs are offered to eligible staff.

Qualifications

• Active Wisconsin SLP license required.
• Two years’ experience preferred.
• CPR certification required.
• Valid Wisconsin driver's license and auto insurance.

Responsibilities

• Provide intermittent in-home speech therapy per physician orders and plan of care.
• Work under supervision of Director of Clinical Services/Clinical Manager.
• Collaborate with care teams to support patient goals.
