Job summary

Reserve Care is seeking a Registered Nurse (RN) to provide terminally ill patient care in Medina County, OH. The role involves implementing the Plan of Care, assessing patient and family needs, and collaborating with the multidisciplinary team to ensure compassionate end-of-life support.

Responsibilities include symptom management, patient education, on-call participation, and coordination with physicians. Ohio RN licensure, CPR, and a valid driver’s license are required; hospice certification

Qualifications

• Licensed RN in the state of Ohio.
• Experience in hospice care, home health, oncology, or pain and symptom management preferred.
• Current CPR certification required.
• Valid driver’s license with good driving record required.
• Hospice and Palliative Care Certification desired; must be obtained within 2 years of hire.

Responsibilities

• Provide prescribed medical treatment and personal care to terminally ill patients.
• Coordinate care plans with the transdisciplinary team.
• Monitor and manage pain and symptoms.
• Educate patients and families on care and health maintenance.
• Participate in care conferences, discharge planning and on-call rotation.
