Job summary

Agape Hospice Care Atlanta is seeking a compassionate Registered Nurse to administer skilled nursing care for hospice patients in their homes across the Snellville, Grayson, and Loganville areas. You will coordinate with the interdisciplinary team, patients, families, and referring agencies to ensure high-quality end-of-life care.

Responsibilities include comprehensive assessments, implementing the plan of care, educating patients and families, maintaining progress notes, and providing on-call

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN in Georgia.
• Two years’ RN experience in a clinical care setting; hospice/home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal, communication, and organizational skills.

Responsibilities

• Perform initial and ongoing assessments of patients' needs and plan of care.
• Coordinate care with the interdisciplinary team and families.
• Educate patients and families about disease, end-of-life care, and self-care.
• Maintain progress notes and documentation.
• Provide on-call nursing service as assigned.
• Coordinate referrals and services as needed.
