Job summary

Compassus is seeking a Hospice Registered Nurse / RN Case Manager to support patients at home in Grand Blanc, MI. You will assess needs, develop care plans, and coordinate services with the patient, family, and care team.

You’ll build trusting relationships, educate caregivers, and help patients spend meaningful time with loved ones while delivering comfort and dignity. A Michigan RN license and hospice experience are preferred.

Qualifications

• Active RN license in Michigan.
• Hospice experience preferred.
• Valid driver’s license with reliable vehicle.
• Current CPR/BLS certification.

Responsibilities

• Provide compassionate nursing care in home hospice settings.
• Educate and empower caregivers in patient care.
• Collaborate with the care team to coordinate services.
• Monitor patient condition and adjust care plans accordingly.
