Job summary

Compassus in Texas is seeking a Hospice Registered Nurse / RN Case Manager to coordinate patient care across the continuum, ensuring comfort and dignity while supporting families.

You will collaborate with a multidisciplinary team, educate caregivers, manage caseloads, and provide compassionate, patient-centered care in home or facility settings.

Qualifications

• RN license in the state you work
• Two years of experience as a registered nurse, hospice experience preferred
• Current driver’s license
• CPR and BCLS certification

Responsibilities

• Coordinate patient care with the multidisciplinary care team.
• Build trusting relationships with patients and families.
• Educate caregivers on at-home care and comfort measures.
• Collaborate to provide peace, dignity, and quality of life for patients.
• Assist with care planning and discharge processes.
