Job summary

Silverado is hiring Hospice LVNs to deliver skilled nursing and compassionate palliative care across San Bernardino, Riverside, and Orange Counties. You will provide pain management, education, and supportive care to patients and families in their homes, under RN supervision.

Qualifications

• Valid LVN/LPN license in the state
• Minimum 1 year of LVN/LPN experience
• CPR certification required
• Hospice experience preferred but not required
• Strong written and verbal communication skills

Responsibilities

• Deliver skilled nursing care under the direction of a Registered Nurse
• Provide pain and symptom management, medical treatments, and patient education
• Support patients and families through end-of-life care with empathy and professionalism
• Participate in care planning and collaborate with the interdisciplinary Palliative team
• Maintain accurate documentation and comply with Palliative protocols
• Travel to patient homes across San Bernardino County, Riverside County and Orange County
