Job summary

Southern Grace Hospice LLC in Sherman, TX is seeking a compassionate PRN Certified Nursing Assistant to provide one-on-one end-of-life care to patients in the community. This flexible role supports families during critical moments and collaborates with the interdisciplinary team to deliver dignified comfort.

Responsibilities include personal care, mobility assistance, observing changes, light housekeeping, and emotional support.

Qualifications

• Active Texas CNA certification is required.
• Hospice or home health experience is preferred.
• Reliable transportation and willingness to travel within the Dallas area.
• Strong communication skills and a compassionate, patient-centered mindset.
• Ability to work independently and adapt to varying schedules.

Responsibilities

• Provide personal care (bathing, grooming, hygiene, toileting, mobility assistance).
• Support patients with daily living activities while maintaining dignity and comfort.
• Observe and report changes in patient condition to the clinical team.
• Assist with light housekeeping related to patient care.
• Offer emotional support to patients and families.
