Job summary

Ambercare Hospice in Belen, NM seeks a compassionate PRN Chaplain to join our hospice team. You will provide spiritual care, emotional support, and presence to patients and families during end-of-life moments.

Working within an interdisciplinary team, you will address spiritual needs, guide bereavement care, and participate in care planning and on-call coverage as needed. We offer flexible PRN hours, mileage reimbursement, and opportunities for growth within Addus HomeCare’s network.

Qualifications

• Graduate of an accredited seminary or theology school; ordination or commission.
• Clinical Pastoral Education (CPE) completed or preferred.
• Hospice experience with death and dying strongly preferred.

Responsibilities

• Perform spiritual bereavement assessments and address needs in the patient’s plan of care.
• Provide direct spiritual care and bereavement counseling to patients and families.
• Document patient services accurately to reflect the plan of care.
• Attend group meetings to coordinate care among interdisciplinary teams.
• Provide education and support to the interdisciplinary group on spiritual care.
• Participate in Continuous Care and on-call as needed.
• Assist in supervising spiritual care volunteers as needed.
• Develop and implement bereavement plans and provide services for up to one year after the patient's death.
