Job summary

Stonerise Home Health is seeking a Speech Language Pathologist to provide evaluation, treatment, and coordination of care in patients' homes. The role emphasizes comprehensive communication assessments and individualized plans of care to improve patients’ speech and language function.

The position requires Master’s in Speech-Language Pathology with internship, CCC-SLP eligibility, and a current license to practice.

Qualifications

• Master’s degree in Speech-Language Pathology with internship.
• CCC-SLP certification per ASHA requirements.
• Licensed to practice Speech-Language Pathology.
• Minimum of 1 year clinical experience in Speech-Language Pathology.

Responsibilities

• Conducting evaluations, assessing home environment, identifying equipment needs relative to speech/language function.
• Identifying functional speech defects and establishing a plan of care to improve patients’ function.
• Administering speech therapy program utilizing specialized therapeutic technique and/or equipment.
• Recommending communication devices/aides as needed.
• Conferring with referring physician and other organization personnel to ensure coordinated and comprehensive care.
• Identifying patient and family/caregiver needs for other services and refers as appropriate.
• Instructing patient, family/caregiver and other organization health care personnel in the patient’s treatment regime as indicated.
• Supervising home health aides in procedures directly related to the speech-language therapy plan of care.

Education, Licensure & Certification Requirements

• Possesses a degree that includes an internship from an approved Master’s program in Speech-Language Pathology
• Meets education and experience requirements for a Certificate of Clinical Competence in Speech-Language Pathology, granted by the American Speech-Language-Hearing Association (ASHA)
• Is certified to practice Speech-Language Pathology
• A minimum of one (1) year of clinical experience in Speech-Language Pathology
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements in good working order
