CommuniCare Family of Companies seeks an in-home Occupational Therapist to assess patient function, evaluate home safety, and develop individualized treatment plans. You will collaborate with physicians and care teams to optimize outcomes and educate patients and families on therapy techniques.

The role requires WV licensure, at least two years of OT experience, and a passion for delivering quality home health services to improve independence at home.

Qualifications

• Must hold a graduate OT degree and be licensed to practice in West Virginia.
• Minimum of two years of clinical OT experience.
• Experience delivering in-home occupational therapy services preferred.

Responsabilités

• Evaluate patients' functional status and independence goals.
• Assess home environment for safety hazards and barriers.
• Contribute to and develop the plan of care for each patient.
• Set goals and establish treatment programs to improve function.
• Communicate plan of care with physicians and team members.
• Educate patients and families on therapeutic activities and use of devices.
• Maintain clinical records and report changes to supervising clinician.
• Assist with prosthetic/orthotic device training when applicable.

• Evaluating patients' functional statuses (muscle function, endurance, visual coordination, written and verbal communication, self-care abilities, etc.)
• Evaluating home environment for hazards/barriers to more independent living and identifying equipment needs
• Participating in the development of the plan of care
• Developing treatment programs and establishing goals for improved function.
• Communicating plan of care to the physician and other team members
• Teaching new skills or retraining patients in once familiar daily activities that have been lost due to illness or injury
• Maintaining appropriate clinical records, notes, and reporting to the physician any changes in patient conditions
• Fabricating splints and instructing patient in the use of various types of adaptive equipment to improve function
• Training patient in the use of prosthetic and/or orthotic devices
• Identifying patient and family/caregiver needs for other home health services and referring as appropriate

Education, Licensure & Certification Requirements

• Graduate of an Occupational Therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Two (2) years of clinical experience as an Occupational Therapist
