Job summary

At Your Side Home Care Services- Willowbrook seeks a compassionate Home Health RN (PRN) to deliver skilled nursing in patients’ homes, utilizing OASIS to develop care plans and maintain meticulous documentation.

Texas RN license required with 1 year of home health experience; wound care, infusion therapy, PEG tube meds, and Kinser EMR experience preferred. BSN not required. 10 hours/week at $40–$50/hour.

Qualifications

• Current Texas RN license.
• Minimum 1 year of home health nursing experience.
• OASIS documentation experience required.
• Wound care and infusion therapy experience.
• Experience administering medications via PEG tube.
• Kinser EMR knowledge.
• BSN preferred but not required.

Responsibilities

• Conduct initial and ongoing patient assessments using OASIS to develop care plans.
• Provide skilled nursing care including wound care and infusion therapy.
• Educate patients and families on health management, medications, and prevention.
• Document patient conditions, interventions, and progress accurately.
• Submit OASIS documentation to Medicare and agency standards.
• Communicate changes in patient status to the care team and implement interventions.
• Serve as liaison between patient, family, and care team.
• Adhere to best practices and evidence-based protocols.
• Demonstrate strong communication to build rapport with patients and families.
• Maintain licensure and pursue ongoing professional development.
