Job summary

Choice Health At Home is seeking a Home Health Field Registered Nurse (RN) in Longview, TX. This full-time role involves comprehensive in-home patient assessments, crafting and updating the plan of care, and educating patients and families to promote independence.

You will collaborate with interdisciplinary teams to deliver high-quality care, manage complex conditions, and ensure smooth coordination with physicians and caregivers in the home setting.

Qualifications

• Current RN License
• Valid Driver's License
• CPR Certification
• Valid Auto Insurance
• At least 2 years of Home Health RN experience preferred

Responsibilities

• Perform initial and ongoing comprehensive assessments and treatment on the patient’s physical, functional, psychosocial, and environmental needs.
• Develop and document the plan of care to ensure quality and continuity of care and revise as necessary.
• Educate patient and family regarding self-care, nutrition, and dietary needs.
• Determine scope and frequency of services based on acuity and caregiver ability.
• Initiate appropriate preventative and rehabilitative nursing procedures.
• Provide directions to Home Health Aide and LPN/LVN to ensure continuity of service.
