Job summary

Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health Physical Therapist (PT) to deliver skilled therapy to patients in south Sarasota County, including Venice, Nokomis, Siesta Key, and Osprey. Schedule is PRN with 5–10+ home health visits weekly.

You will perform comprehensive assessments, initiate and revise plans of care, and provide rehab services and education to patients and families.

Qualifications

• Active Florida PT license.
• Minimum 1 year experience as a licensed PT.
• Home health care experience preferred.
• Strong assessment, critical thinking, and patient education skills.
• Willingness to participate in weekend and holiday on-call rotations.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled occupational therapy services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care
