Job summary

Cleveland Clinic Home Health Services is seeking a PRN Home Health RN to join the West-Cuyahoga County team. You will deliver high-quality care to patients in the comfort of their homes, coordinating with an experienced Home Care team.

Qualifications

• Graduate from an accredited school of Professional Nursing.
• Current state licensure as a Registered Nurse (RN).
• Basic Life Support (BLS) Certification through the American Heart Association (AHA).
• One year of clinical experience as an RN in an acute medical/surgical setting.
• Six months of experience as a Homecare Nurse.
• Valid Driver’s License (VDL) in Ohio and proof of automobile insurance.

Responsibilities

• Provide direct nursing care in accordance with established policies and protocols.
• Implement and monitor patient care plans.
• Provide supervision to LPNs, Home Care Aides and families involved in the care of the patient.
• Perform admission assessments and reassessments within required time frames.
• Assess patient safety risks and coordinate care with the healthcare team.
• Escalate changes in patient condition to the physician as appropriate.
