Job summary

ProHealth Home Health and Hospice is seeking a part-time Registered Nurse to administer skilled nursing care in patients' homes in Addison, TX. The role follows physician orders and a formal care plan under supervision.

The RN will perform initial and ongoing assessments, develop and modify care plans, and collaborate with the care team. Strong nursing experience, English proficiency, and initiative are essential for success.

Qualifications

• Graduate of an approved school of professional nursing with active state license as registered nurse, or a multi-state license issued by an NLC member state.
• Ability to read, write, and comprehend English.
• Two (2) years’ nursing experience.
• Acceptance of philosophy and goals of this organization.
• Ability to exercise initiative and independence.

Responsibilities

• Provides services in accordance with the plan of care.
• Makes the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
• Initiates the plan of care and necessary interventions.
• Provides services requiring substantial specialized nursing.
• Initiates preventive and rehabilitative nursing.
• Prepares clinical and progress notes for each patient visit and summaries of care conferences.
• Coordinates patient care with other team members.
• Informs personnel of changes in condition and patient needs.
• Counsels patient and family in meeting nursing and related needs.
• Participates in inservice education and training.
• Adheres to agency policies and procedures.
• Processes orders and notifies physician of patient needs; completes certification/recertification orders and discharge summaries.
• Determines the amount and type of nursing care needed for each patient.
• Refers patients to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker as needed.
• Supervises and teaches other nursing staff.
• Conducts patient care conferences for assigned patients.
• Participates in peer review and QA/PI activities.
• Provides total patient care.
• Takes on-call duty nights, weekends and holidays as required.
• Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant condition changes promptly.
• Uses ICD-10 codes appropriately.
