Job summary

Qualifications

• Current RN license, state(s) licensed to practice.
• Valid driver's license with reliable transportation and liability insurance.
• Current CPR certification.
• 1+ year of clinical RN experience; approval by VP if less than 1 year.
• RN Intern program required if <6 months RN experience.

Responsibilities

• Coordinates and oversees all aspects of patients' home care.
• Collaborates with patient care teams to develop and implement care plans.
• Educates families on patient condition, needs and care.
• Performs head to toe physical assessments.
• Checks vital signs and manages medications.
• Gives injections, IV therapy or tube feedings.
• Administers and maintains catheters.
• Completes documentation timely and accurately.
• Supervises LPNs and HHAs.
• Regularly communicates patient progress to the clinical manager, physician and care team.
• Other duties as assigned.
