Job summary

Amedisys is seeking a PRN Per Diem Registered Nurse to cover the Cape Cod region on an on-call basis. You will perform admissions assessments and manage patient care in home settings, collaborating with the clinical team to ensure continuity and quality care.

The role offers flexible per diem hours, competitive hourly pay, and access to comprehensive benefits while supporting patients in their homes. Prior hospice/home health experience is a plus, with a focus on compassionate, patient-centered

Qualifications

• Current RN license for the state(s) you work.
• 1 year nursing experience preferred; new graduate RN program available.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Completes and revises the plan of care with the care team.
• Communicates patient progress to the clinical manager and physician.
• Maintains patient care records with accuracy and timeliness.
• Records changes in pain and symptom management as needed.
• Educates families on patient condition and care needs.
• Makes referrals to other hospice disciplines when appropriate.
• Performs on-call responsibilities as assigned.
• Performs other duties as assigned.
