Job summary

Paradigm Home Health Companies is seeking a Physical Therapist to provide direct in-home patient care on an as-needed (PRN) basis. You will develop and revise plans of care with physicians and care teams, deliver therapeutic treatments, and document progress.

Requirements include a current Texas PT license, CPR certification, and a valid Texas driver’s license with auto insurance. Knowledge of WellSky EMR is a plus; mileage reimbursement is provided per visit.

Qualifications

• Current Texas PT license required.
• Current CPR certification required.
• Valid Texas driver’s license with automobile insurance.
• Knowledge of WellSky EMR software is a plus.

Responsibilities

• Develops and revises plan of care in consultation with physician and other careteam members.
• Provides therapeutic treatments and evaluation of equipment needs as applicable.
• Participates in clinical conferences and inservice programs.
• Conducts onsite supervisory visits of physical therapy assistants.
• Advises and consults with the family and other agency personnel as applicable.
• Prepares clinical and progress notes.
• Protects and honors patient and coworker confidentiality and right to privacy.
• Evaluates outcomes of care.
• Plans for discharge.
