Job summary
PARADIGM REHAB & HEALTH CARE LLC in Keller, Texas is seeking a Physical Therapist PRN to provide direct patient care to home-bound patients. The role includes developing care plans, conducting evaluations, and providing therapeutic treatments.
This position requires a current Texas PT License, CPR certification, and a valid driver’s license. Pay will be per visit, including mileage reimbursement and additional points assigned for drive time.
Qualifications
- Must have a current Texas PT License.
- Current certification in CPR is required.
- Valid Texas Driver's License and Automobile Insurance are necessary.
Responsibilities
- Develops and revises plan of care in consultation with physician.
- Provides therapeutic treatments and evaluation of equipment needs.
- Conducts on-site supervisory visits of physical therapy assistants.
- Prepares clinical and progress notes.
- Evaluates outcomes of care.
