Responsibilities:

  1. Develop therapy treatment plans for home health patients
  2. Implement standardized tests for accurate diagnosis of physical, cognitive and communication disorders
  3. Establish treatment goals and milestones with patients
  4. Timely documentation of treatment plans and progress reports
  5. For PTs and OTs, be able to supervise PTAs and COTAs
  6. Update and communicate patient progress to relevant parties
  7. Research related physical and functional problems

Qualifications:

  1. Current California Professional License required
  2. 2 years of clinical experience as a therapist is required (including other settings outside of home health)
  3. Previous experience in home health setting is preferred but we are willing to train
  4. Possesses and maintains current CPR certification
  5. Must be a licensed driver with automobile that is insured in accordance with state requirements and is in good working order

  • Meets physical requirements for clinical aspects of the position.
  • Ability to build rapport with patients
  • Excellent written and verbal communication skills
  • Positive and patient demeanor
  • Effective verbal and written communication skills, including response to a variety of stressful situations and/or emergency patient incident.
  • Strong organizational skills and leadership ability
  • Basic Computer And Typing Skills Are Required.
