Reliant at Home in Rockwall, TX is seeking a PRN Physical Therapist to support home health services in Rockwall and surrounding areas.

You will perform clinical assessments, develop and revise Plans of Care, document progress notes, and coordinate with the care team while promoting infection control and patient safety.

Qualifications

• Licensed Physical Therapist in Texas with current license.
• Home health experience strongly preferred.

Responsabilités

• Perform clinical assessment and identify patient needs and appropriateness of care.
• Document and complete a Plan of Care, progress notes, and other clinical records.
• Oversee team members when applicable to deliver patient care aligned with patient needs.
• Apply infection control and universal precautions in patient care.
• Plan and provide treatment for neuro-muscular, neurological and orthopedic disorders based on referrals.
• Meet continuing education requirements as required by the agency/licensing board.
• Instruct patients and families on transfer techniques and safe use of equipment.
