Qualifications

• Home health experience strongly preferred.
• Ability to perform clinical assessments and develop a Plan of Care.
• Experience with neuro-muscular, neurological and orthopedic therapies.
• Strong documentation skills for care plans and progress notes.

Responsibilities

• Perform clinical assessment; identify patient needs and appropriateness of patient care.
• Document, prepare and complete a Plan of Care, progress notes, and other clinical records; revise POC as needed.
• Oversee other team members when applicable in patient care.
• Apply infection control and universal precautions in care activities.
• Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders per medical referral.
• Meet continuing education requirements of the agency/licensing board.
