Qualifications

Responsibilities

• Conduct initial assessments of patients’ physical abilities and develop treatment plans.
• Provide therapy services to patients in their homes.
• Monitor patients’ progress and adjust treatment plans as needed.
• Communicate with patients’ physicians and other healthcare professionals to coordinate care.
• Document patients’ progress and maintain accurate records.
• Provide education to patients and their families on how to manage their conditions and prevent further injury.
