Job summary

Frontpoint Health is seeking a licensed Physical Therapist (PT) to provide home health services in the Dallas, TX area on a PRN/per visit basis. The PT will assess patients, develop treatment plans, and deliver therapy in patients’ homes, coordinating with physicians and healthcare professionals to optimize outcomes.

The role requires strong communication, the ability to work independently, and a commitment to patient education and safety.

Qualifications

Current license to practice as a Physical Therapist in the state of Texas.
Minimum of 1 year of experience in home health.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
Strong problem-solving and critical thinking skills.
Valid driver’s license and reliable transportation.

Responsibilities

Conduct initial assessments of patients' physical abilities and develop treatment plans.
Provide therapy services to patients in their homes.
Monitor patients' progress and adjust treatment plans as needed.
Communicate with patients' physicians and other healthcare professionals to coordinate care.
Document patients' progress and maintain accurate records.
Provide education to patients and their families on how to manage their conditions and prevent further injury.
