  Job Summary and Qualifications

  The Physical Therapist (PT) delivers high-quality, patient-centered care by managing therapeutic processes for assigned patients, coordinating with physicians and the care team. Responsibilities include patient assessments, developing and implementing care plans focused on goals, engagement, and positive experiences. The PT provides or directs most of the patient’s physical therapy throughout the care continuum.

  Key Responsibilities:

  • Provide evaluations and treatment within legal and ethical standards.
  • Assess and evaluate therapeutic, functional, and rehabilitative status; develop care plans.
  • Deliver purposeful, skilled therapy at patients' homes, using appropriate procedures and techniques.
  • Educate patients, families, and caregivers on therapies, exercises, and devices for outside sessions.
  • Prioritize caseload, coordinate staffing, and manage assignments.

  Qualifications:

  • Licensed Physical Therapist
  • Valid Driver's License
  • Bachelor's Degree

  Experience: No prior experience required.
