Position Summary:

The primary purpose of this job is to return patients to optimal physical function and facilitate achievement of patient, physician, and therapy goals. Follow all health, sanitary, and infection control policies, and maintain standards of practice as set by the community’s administration and Policies and Procedures.

Education & Responsibilities:

• Possess a Bachelor’s or Master’s Degree in Physical Therapy from an accredited college or university
• Hold a current and valid license from the state of employment
• Screen and evaluate residents to determine the need for Physical Therapy
• Establish individualized treatment programs in consultation with facility staff
• Provide evaluation results and treatment objectives to caregivers, family, and residents
• Complete all documentation within the designated time frame
• Implement and monitor treatment plans, adjusting as necessary
• Supervise Physical Therapy Assistants and Rehab Aides

Experience:

Previous experience in long-term care is preferred but not required.

Shift and Wage:

Full-Time, Part-Time, and PRN opportunities available.
