Job summary

Advantage Home Health is seeking a compassionate Occupational Therapist to join our Home Health team in Mifflin County, PA for a PRN role with daylight hours and flexible scheduling. You will provide one-on-one care in patients' homes, helping individuals regain independence and confidence in daily activities.

You'll evaluate patients, develop personalized treatment plans, document progress, and collaborate with physicians and nurses to optimize outcomes.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Pennsylvania OT license.
• CPR certification (or ability to obtain upon hire).
• Valid driver's license and reliable transportation.
• Home health experience preferred, but not required.

Responsibilities

• Provide skilled occupational therapy services in accordance with physician orders and individualized plans of care.
• Evaluate patients and develop personalized treatment programs to improve function, safety, and independence.
• Assess patients' ability to perform activities of daily living (ADLs) and recommend adaptive techniques or equipment as needed.
• Document patient evaluations, treatments, and progress in a timely and accurate manner.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure optimal patient outcomes.
• Educate patients and caregivers on home safety, adaptive strategies, and ongoing therapy goals.
• Supervise Certified Occupational Therapy Assistants (COTAs) as appropriate.
