Job summary

CarePartners Health Services in Asheville, North Carolina, is seeking a Registered Nurse for home health. You will assess patients in the home, complete OASIS paperwork, and coordinate with physicians to ensure quality care.

The role involves supervising home health aides and LPNs/LVNs, and participating in on-call rotations per agency policy. An RN license and a valid driver's license are required, with an associate or bachelor's degree in nursing preferred.

Qualifications

• RN license is required to practice as a home health nurse.
• Valid Driver's License is required to travel to patient homes.
• Associate or Bachelor's degree in nursing or related field.

Responsibilities

• Assesses home care patients identifying physical, psychosocial and environmental needs.
• Completes OASIS and assessment paperwork according to agency policy.
• Communicates significant findings to Clinical Manager and physician.
• Supervises and provides direction to home health aides and LPNs/LVNs.
• Participates in on-call rotation and emergency calls as per policy.
