Discovery At Home LLC is seeking an on-call PRN Registered Nurse to provide comprehensive home health care across Florida. The role requires active state licensure, CPR certification, and familiarity with Medicare/Medicaid guidelines.

Qualifications

• Must hold an active nursing license in Florida without restrictions.
• One year of home health experience preferred.
• Current CPR certification is required.
• Strong communication and documentation skills.
• Proficiency with basic computer tasks and Windows applications.

Responsibilities

• Provide comprehensive nursing care to patients following nursing standards and agency policies.
• Assess patient needs and develop individualized care plans to ensure optimal outcomes.
• Administer medications, treatments, and therapies as prescribed.
• Monitor patient progress and document vital signs and changes in condition.
• Collaborate with the healthcare team to coordinate care and ensure continuity.
• Educate patients and families about treatment plans, medications, and self-care techniques.
• Respond to emergencies and perform life-saving interventions as needed.
• Maintain accurate medical records and ensure infection control practices.
